A 23 year old female farmer by occupation presented to OPD with chief complaints of
Headache since 10 days
Inability to pass urine since 10 days
Fever since 5 days
Vomiting since 5 days
Patient is a rice field farmer and a mother of 2 children. She studied till 10th standard. She got married at the age of 16. She had her first pregnancy at the age of 17
Patient was apparently asymptomatic 10 days ago then she developed diffuse headache which was sudden in onset dragging type radiating towards neck which aggravated on talking, rotating the head and relieved on taking medication, it is associated with nausea and phonophobia.
